Insurance
I am a participating provider with Blue Cross Blue Shield and APS Healthcare. In order to determine whether I am in-network with your plan, you can call the member services number on the back of your insurance card, provide them with my name, and let them know that I participate with the local plan in. You may also want to obtain some additional information about your mental health benefits by asking the following questions:
- Is a referral from a primary care physician required?
- Do I need pre-authorization before seeing a mental health provider?
- Are my mental heath benefits carved out to another carrier?
- Do I have a deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- Is there a co-pay and/or co-insurance?
- Do I have out-of-network mental health benefits?
- What is my deductible and has it been satisfied?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment
Payment by cash, check or major credit card (AMEX, Visa, Discover or Master Card) is expected at the end of each session.
Cancellation Policy
Please notify me within 24 hours if you cannot attend your appointment. Late cancellations incur the full cost of the unattended session.
